Sports Activity Waiver

Activity Waiver & Sign-Up

In consideration of being allowed to participate in any way in New Hope Community Church (London) athletic programs including LifeGroups, related events or activities, the undersigned acknowledges, appreciates, and agrees that:
 
  1. I acknowledge that I am willingly choosing to participate in athletic activities that may potentially result in damage to personal property, serious injury, disability or death. I further acknowledge that there may be other risks not easily foreseeable at this time. I hereby assume full responsibility for all the known and unknown risks and waive all claims of injury to my body or damage to my property. 
 
  1. I hereby accept all risks associated with participating in an athletic activity through New Hope Community Church (London). I release and forever discharge New Hope Community Church, its employees, directors, officers, volunteers, and legal agents for any and all responsibilities, damages or other liability resulting from or in any way related to my participation in any athletic activity offered through New Hope Community Church (London) including but not limited to the negligence of the organization. I assume all financial and legal responsibility for any injury or injuries suffered during participation in the above-mentioned athletic activity. 
 
  1. My mental and physical condition is appropriate to participate in the activity listed above and I assume all risks related to this. It is my sole responsibility to assess whether any activities are too difficult for me. By commencing an activity, I acknowledge and accept the suitability and conditions of the activity. 
 
  1.  I certify that I have read this document, and I fully understand its content, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, spouse, children, parents, guardians, next of kin, executors, administrators and legal or personal representatives. I am aware that this is a release of liability and a contract, and I sign it on my own free will. I further acknowledge that by signing this agreement I have waived my right to maintain a lawsuit against New Hope Community Church (London) on the basis of any claims from which I have released herein.
